Attention: All students entering the 7th grade must have two additional shots. For you to comply with the State of Tennessee, we need proof of these shots from the health department or your local health care provider before the start of the 2024-25 school year.  All immunizations must be on the TN Department of Health form - any other version will not be accepted per school board policy. Your 7th grader will not be allowed to attend school after the deadline if this form has not been submitted per state law.

New students are also required to have proof of current immunizations on the same form.  We don’t request immunizations from previous schools, guardians must supply a current certificate at enrolllment.
